Supporting Information (Normally required before this priority can be awarded).

Band 1 A move is necessary to enable a disabled/mentally ill/elderly person and carer to be close in order to prevent immediate admission to hospital or a home where essential support is given. Will only apply where the applicant is looking to move to the same estate as carer; A letter or email from a medical professional working with the person in question which confirms that the care provided to or from the applicant will prevent immediate admission to hospital or a home.
Band 1 Violent injury/death, suicide, rape or attempted rape in or near the home which directly affects the applicant or a member of the applicants’ household A letter or email from the Police which confirms what has occurred, that they support the move and that they feel there is on-going risk.
Band 1 Where a neighbourhood dispute/racial harassment/hate crime incident results in court action and the applicant needs to move away from an area; A letter or email from the Police which confirms what has occurred, that they support the move and that they feel there is on-going risk and confirmation that court action is ongoing.
Band 1 Life threatening violence or the likelihood of it. A letter or email from the Police which confirms what has occurred, that they support the move and that they feel there is on-going risk.
Band 1 Children being returned from care but dependent on a change of accommodation. Letter from social care confirming a child is being returned from care but this is dependent on a change of accommodation.
Band 1 Freeing a hospital bed or residential home placement where present accommodation is deemed unsuitable following professional advice. Letter from a medical or social care professional confirming that the customer is in a hospital bed or residential home placement and confirming that their present accommodation is unsuitable.
Band 1 Change of accommodation required for exceptional social/medical factors where moving would enable the delivery/monitoring of essential support services. Awarded when Housing Support has confirmation that the applicant is engaging with supporting agencies. Supporting letters from Social care confirming exceptions factors exist and applicant is engaging with support.
Band 2  Burglary: Where the applicant is considered vulnerable and is the target of repeat offending. A letter or email from the Police which confirms what has occurred, that they support the move and that they feel there is on-going risk.
Band 2  Neighbourhood disputes/racial harassment incidents/hate crime resulting in intimidation or serious threats or damage to property which are recurrent. A letter or email from the Police which confirms what has occurred, that they support the move and that they feel there is on-going risk.
Band 2  To move away from criminal activity and/or drug problems where the applicant would be vulnerable to being drawn back to previous criminality. A letter or email from social care, police or probation confirming the customer needs to move away from criminal activity and/or drug problems to avoid them being drawn back in previous criminal activity.
Band 2  Couples/Families/Single people in financial hardship actively looking to downsize from larger accommodation to smaller accommodation and circumstances currently assessed as band 2. A letter or email from social care, social landlord or another support agency which confirms financial hardship. Customer must be downsizing to a smaller property. 
Band 2  Applicant is a current or former member of the armed forces, injured serviceman or woman, bereaved spouse or civil partner or existing or former member of the reserved forces who is suffering from a serious injury, illness or disability which is attributable (wholly or partly) to their service (families). Supporting information from Ministry of Defence or support agency confirming service and confirming that the applicant has suffered from a serious injury, illness or disability which is attributable (wholly or partly) to their service.
Band 3 Accident or bereavement in or near the home which is seriously affecting the applicant or a member of their family. A letter or email from police, social care or a support agency which states that there has been an accident or bereavement in or near the home which is seriously affecting the applicant or a member of their family.
Band 3 Neighbourhood disputes/racial harassment resulting in graffiti, gestures, letters. A letter or email from the ASB Team, Victim Support or the Police which confirms what has occurred, that they support the move and that they feel there is on-going risk.
Band 3 Where, following the breakdown of a marriage, civil partnership or established common law relationship the former partners are still living in the same property. A letter or email from both parties confirming that relationship has broken down, evidence any benefits are now being claimed separately and that any joint claims had been ended, supporting letters or emails from support agencies confirming the relationship has broken down may also be considered.
Band 3 Couples/Families/Single people in financial hardship actively looking to downsize from larger accommodation to smaller accommodation and circumstances currently assessed as band 3. A letter or email from social care, social landlord or another support agency which confirms financial hardship. Customer must be downsizing to a smaller property.

Supporting information from Ministry of Defence or support agency confirming service and confirming that the applicant has suffered from a serious injury, illness or disability which is attributable (wholly or partly) to their service.
